Antimicrobial resistance in Neisseria gonorrhoeae and Treponema pallidum: evolution, therapeutic challenges and the need to strengthen global surveillance

چکیده
INTRODUCTION The emergence of drug resistance among bacterial, viral and protozoan sexually transmitted infections (STI) in recent years threatens to undermine global STI control programmes. While this review will focus on microbiological resistance determinants for two important bacterial STI pathogens, namely Neisseria gonorrhoeae and Treponema pallidum, it is important to appreciate that a number of nonmicrobiological determinants may also directly influence both the emergence and transmission of antimicrobial resistant STI pathogens (table 1). This short paper describes the historical development of antibiotic resistance in N gonorrhoeae and T pallidum, outlines the challenges of identifying and treating these resistant infections, and highlights the requirement for the strengthening of global microbiological surveillance programmes for STI.

The role of antimicrobial resistance monitoring in Neisseria gonorrhoeae
Treatment regimens for most sexually transmitted diseases of bacterial origin are well established. For example, treatment of infections with Chlamydia trachomatis is usually now with macrolides, and Treponema pallidum is still reliably susceptible to penicillins. However, antibiotic treatment for gonorrhoea is more complicated because of the propensity of the gonococcus to develop antimicrobia...

PURPOSE OF REVIEW This review provides an update on the need, development status, and important next steps for advancing development of vaccines against sexually transmitted infections (STIs), including herpes simplex virus (HSV), Neisseria gonorrhoeae (gonorrhea), Chlamydia trachomatis (chlamydia), and Treponema pallidum (syphilis). RECENT FINDINGS Global estimates suggest that more than a m...
متن کاملAntimicrobial resistance in Neisseria gonorrhoeae: history, molecular mechanisms and epidemiological aspects of an emerging global threat
Neisseria gonorrhoeae is the agent of gonorrhea, a sexually transmitted infection with an estimate from The World Health Organization of 78 million new cases in people aged 15-49 worldwide during 2012. If left untreated, complications may include pelvic inflammatory disease and infertility. Antimicrobial treatment is usually effective; however, resistance has emerged successively through variou...

OBJECTIVE To perform a European sentinel surveillance study for antimicrobial resistance (AMR) in Neisseria gonorrhoeae as part of the European Surveillance of Sexually Transmitted Infections Programme. METHODS From 2006 to 2008 17 countries participated in the AMR surveillance programme. The susceptibility of a total of 3528 consecutive isolates was tested using the agar dilution breakpoint ...
متن کاملStrengthening Global Surveillance for Antimicrobial Drug–Resistant Neisseria gonorrhoeae through the Enhanced Gonococcal Antimicrobial Surveillance Program
Monitoring trends in antimicrobial drug-resistant Neisseria gonorrhoeae is a critical public health and global health security activity because the number of antimicrobial drugs available to treat gonorrhea effectively is rapidly diminishing. Current global surveillance methods for antimicrobial drug-resistant N. gonorrhoeae have many limitations, especially in countries with the greatest burde...
